How to request a transcript


A transcript is a certified official copy of a student’s permanent academic record. By law it cannot be released to anyone other than the student without the student’s written permission. The written permission must identify by name the person authorized to obtain the transcript and must have the student’s original signature.

Some things to know before you order a transcript:


  • A transcript picked up by or sent directly to a student is always stamped “issued to student” and placed in a sealed envelope. Not all institutions will accept this type of transcript as official.

  • If a hold has been placed on the student’s record by any USM department (business office, financial aid office, library, athletics) a transcript will not be issued.

  • We cannot accept a transcript request by phone or in the body of an email.  You can complete, sign and scan the form below and email it to us as an attachment.

  • There is a $10 processing fee for all official transcripts.

  • Current students must indicate if the transcript should be held for semester grades or until his/her degree is posted.

To request a transcript, please print and complete this form and submit: 

  • by fax to 913-758-6218 — a faxed request must include your credit card number and expiration date

  • in person
