Completing Required Forms
for all Athletes and all Residents at the University of Saint Mary

USM uses a system called Athletic Trainer System.
To complete your forms online:

1. 1.    Open your internet browser (Internet Explorer seems to work best)
   2.    Enter stmary2.atsusers.com in the address (no www, etc.)
   3.    Enter “new” for the user ID
   4.    Enter “new” for the password
   5.    Click the “login” button
   6.    Fill out the information to register
   7.    Click save
   8.    More tabs should appear now
   9.    Go to Medical History
   10. Fill out surgery history
   11. Click edit to enter new surgical history
   
   
2. 1.    Click save after each new entry
   2.    Fill in the Medical History (please provide details in the appropriate box for any question you answer “yes” to)
   3.    Click Save
   4.    Go to Immunizations
   5.    Click edit to add new type
   6.    Click save after each new entry
   7.    Click on the Insurance Tab
   8.    Click edit to begin entering information
   9.    Fill out insurance information(adding a new insurance company if necessary)
   10. Click save to save all your information
   11. Do the same with the Contacts tab 
   12. Click on Athlete Forms
   13. IF YOU ARE AN ATHLETE: On the drop-down select athletic care policies and press the NEW button, read through and type your name to electronically sign
   14. Click Save
   15. IF YOU ARE A RESIDENT: On the drop down menu select resident accident insurance, read through and if you wish to waive select yes and type your name to electronically sign.
   16. Click Save
   17. Click on a menu button
   18. Click on electronic documents
   19. Click and print physical form if you have not had one in the past two years or after June 1, 2009
   20. Scan and upload completed physical form and a copy of the insurance card
